Pharmacy Team Shortlisted for a National Sustainable Healthcare Award

The Western Trust Pharmacy Team has been selected as a finalist in the national AbbVie Sustainable Healthcare Awards 2017.  The Team, in collaboration with the Northern Health and Social Care Trust (Northern Trust) pharmacy team have been shortlisted in the ‘Making Health and Social Care Patient Friendly’ award. The Team has developed its medicines service for older people in care homes and intermediate care beds.

Part of the team’s work was the development of a Consultant Pharmacist led medicines case management service for older people passing through the hospital setting into a reablement care setting with post discharge follow-up in the Western Trust.  This model of care was then successfully introduced into the Northern Trust with excellent outcomes for patients.  Additionally the Northern and Western Trust Pharmacy Teams have co-developed and provided medicines optimisation outreach services to patients residing in nursing and residential care homes.

Dr Bob Brown, Director of Primary Care and Older People’s Services for the Western Trust said: “I am delighted that the Pharmacy team have been shortlisted in this year’s national Sustainable Healthcare Awards 2017 and wish them good luck at the final later this month. The Pharmacy Team has developed exceptional improvements in pharmaceutical care and innovation that demonstrate clear benefit to the lives of our older people.”

Dr Ruth Miller Regional Project Manager, Medicines Optimisation in Older People at the Western Trust said:  “We are delighted to have been shortlisted for this national award. This award entry seeks acknowledgement of the efforts of a team who delivered an exceptional and innovative service to older people in care homes and intermediate care”.

Anne Friel, Regional Project Lead and Head of Pharmacy and Medicines Management at the Trust said: “The Team has worked very hard to provide a service that focuses on each individual patient’s needs. We have been able to build on this and are now developing this Consultant Pharmacist-led approach across all five Trusts in Northern Ireland.”

Zoe Horwich, Head of Sustainable Healthcare Initiatives at AbbVie UK, said: “There is great work going on across the NHS in making services easier for patients and more effective. We hope that by raising the profile through these awards we can help great ideas to spread. Getting shortlisted to the finalist stage for this year’s competition by our independent panel of judges is an indication of the importance of the work the Western Trust has been doing to improve pharmacy services for older people.”

The Pharmacy team are among three finalists shortlisted in the category and the winners are decided by category judges, who are leaders in the healthcare system. The awards ceremony will take place on the 28 November 2017 in the Kings Fund, London.
