Freedom of Information Form

The Freedom of Information (FOI) Act 2000 means that the public can access information about the Trust. This law, which came into force on 1 January 2005, means that anyone, anywhere can ask for any information we hold.

The FOI Act provides access official information held by the Trust (e.g. corporate or business information) and not to information of a personal nature.  Requests for personal information will be dealt with under the Data Protection Act.  For more information about obtaining your own personal information - click here.

responding to reasonable requests for official information
  
All FOI requests must be submitted in writing and clearly describe the information required. If you want to make a Freedom of Information request, please email:

We would ask that you do not use this form to request personal information. Information of a personal nature cannot be accessed under the FOI Act. See above link about accessing your personal health and social care records under the Data Protection Act.
