Gransha Hospital

GRANSHA HOSPITAL

Gransha Hospital is made up of admission wards which cater for clients who require hospital admission when community treatment is not considered appropriate. The normal age range of clients admitted is between the ages of 16 to 65 years of age (Adult Mental Health).

The Female Clinic and Male Clinic are 17 bedded Acute Admission Wards with 3 integrated Intensive Care Beds, a total of 40 beds. The single sex units consist of one for male and one for female clients from the five catchment areas.

PHILOSOPHY

The Admission Wards aim to treat people as individuals, respecting their rights to dignity and autonomy. Clients will be treated through a multi-disciplinary team approach, with the ultimate aim of returning the client to the community environment as soon as possible.

AIMS

  • To provide a safe, structure environment in which clients can increasingly assume responsibility for themselves.
  • To treat people as individuals, respecting their rights to dignity and autonomy.
  • To provide an atmosphere conducive to the care of people with mental health problems.
  • To assess, plan, implement and evaluate care on an individual basis in order to aid the return of the person to a suitable environment in the community as soon as possible.
  • To promote mental well being.
  • To provide education on mental illness/well-being to carers and clients.


DESCRIPTION OF WARD

The facilities comprise of:

  • Observation areas
  • Sitting rooms
  • Smoking room
  • Bathroom/Shower facilities
  • Toilet areas
  • Group room
  • Bedrooms
  • Dinning area
  • Relaxation room

NAMED NURSE

Each client will have an identified named nurse and an associate nurse allocated to them on admission. The named nurse shall be a qualified first level nurse with a minimum of six months experience.

Responsibility include:

  • Make themselves known to the client, relatives and carers.
  • Ensure a comprehensive assessment of needs is carried out; a care plan based on the assessment is completed and implement; regular evaluations of the care plan and the clients needs are carried out.
  • Ensure the client is involved in the care planning process
  • Ensure that regular mental state assessments are carried out and documented.
  • Ensure that clients are provided with information on their diagnosis and treatment.
  • Spend structured time each day with their clients.
  • Work closely with medical and other disciplines to ensure high quality of care is provided.
  • Plan/Co-ordinate discharge/transfer to another facility.

VISITING

The wards operate an open visiting policy in order to help clients settle into the ward environment. However, some restrictions must be imposed in order to help maintain a therapeutic environment.


  • No visitor shall be allowed on the ward if they have, or are suspected of having taken drugs or alcohol.
  • Visitors should conduct themselves in an orderly manner and ensure that any children accompanying them do likewise.
  • Relatives / friends should not visit during times when a client is scheduled for therapy or at meal times.
  • Visitors shall not be permitted to visit after 9pm, without the permission of the nurse in charge.
  • In the interests of other clients, visitors should restrict themselves to designated visiting areas and adhere to the hospital smoking policy.
  • Facilities are available for visitors to obtain beverages in the snack room.

INTEGRATED INTENSIVE CARE BEDS


PHILOSOPHY


The philosophy of the Intensive Care Beds is centred on recognising the patient as an individual and responding to his/her physical, psychological, social and spiritual needs.

Where possible all aims and goals of care are negotiated with the patient while relatives and cares are involved where appropriate.

AIMS

  • To provide the care which the patient cannot provide for him/herself at the time.
  • To provide intensive and individualised treatment programmes of care for each patient. Appropriate methods of assessment and monitoring will be used to facilitate the evaluation of treatment programmes.
  • To provide and maintain a safe physical environment for the patient.
  • To provide a safe structure in which the patients can increasingly assume responsibility for themselves and their behaviour.
  • To return responsibility for the patient to the catchment area team as soon as it is reasonable to do so.

ADMISSION CRITERIA

  • Patients should normally be aged 16 to 65 years old.
  • Patients over the age of 65 years will need special consideration and assessment of their physical condition prior to acceptance by the Intensive Care Unit.
  • Individuals who require third level supervision as defined in “Levels of Observation/Supervision”.
  • All patients who represent a risk to themselves or others within an open environment due to their illness.
  • Admissions should normally be discussed with the Consultant.

DISCHARGE/TRANSFER OUT CRITERIA

  • When it is deemed that the patient is ready for discharge/transfer from the ICU, representatives of the patients multi-disciplinary team and other agencies, as appropriate, will be invited to participate in the review of the patients; condition and to agree a future care plan.
  • Following Transfer or discharge from ICU, patients may be referred for further care and treatment when the facility of a secure environment is needed again.

PRINCIPLES OF TREATMENT

All treatment should:-

  • Be primarily for the benefit of the patient. Its main aim should be, so far as is possible, to improve health and reduce handicaps, including social handicap.
  • Protect the safety of the patient and others. In the course of treatment or in the interest of safety, restriction of liberty may be necessary but should never be used as a punishment.
  • Respect the patient’s dignity, privacy and rights to basic requirements.
  • Ensure that the patient knows his/her Key Worker/Primary Nurse and their role.

CARE ENVIROMENT

The ward atmosphere will be safe and care with non-violent episodes accepted as the norm. It will be patient centred.
