COMMUNITY PROFESSIONALS

FAMILY DOCTOR

Your GP will offer advice on any Medical problems and will also refer you to any other appropriate professional. Contact: Local Health Centre

SOCIAL WORK SERVICE

Social Workers/ Assistants provide a range of services aimed primarily at assisting you to remain in your own home. These services include:

  • A counselling service.
  • Arranging practical home supports including assistance with both personal and household care. Arranging day care.
  • Assessing social and financial needs,
  • Arranging respite periods in both residential and nursing homes.
  • Acting as an advocate for you by liaising with other agencies eg, DHSS, Housing Executive and Voluntary Bodies.

CARE MANAGEMENT

If your needs are complex and/ or you require a high level of community support and/ or you feel your needs could only be met in a Nursing or Residential Home then a Care Manager will be appointed by the Western Health and Social Care Trust to assess the best form of care and support for you. Requests for a care manager may be made by yourself, carer or any Health or Social Care Professional.

COMMUNITY NURSING SERVICE

This includes both District Nursing and Health Visiting. The nursing service provides a range of both general and specialist nursing care in the community.

This includes: general district nursing care, health education, nursing care for the terminally ill including night time care, nursing aids, general nursing support services, arranging hospital respite.

RESIDENTIAL AND NURSING HOMES

FOR THOSE CONSIDERING ADMISSION TO NURSING OR RESIDENTIAL HOMES

Application to any Home should be made through Social Services, Western Health and Social Care Trust, Community Services Department, Coleshill Road, Enniskillen Telephone: 028 6634 4000
A Care Manager will ask all relevant professionals (eg, GP, District Nurse, Social Worker) for their individual reports in order to assess if the applicant requires a nursing or residential home or if they could be supported in their own home.

FINANCIAL ASSESSMENT

If a Residential or Nursing Home is required then a MEANS TESTED Financial Assessment will be carried out by the Care Manager. This Care Manager will be able to give you details of the outcome of this assessment. A brief summary of this assessment is as follows:
Those with £16,000 or more, in savings property or capital, will have to pay the full weekly cost of the home. However special rules apply for those whose spouse or dependent child remains living in their home. Those who pay privately for their care in a nursing or residential home will continue to be entitled to the State Pension and Attendance Allowance (if applicable).

Where an individual has between £10,000 - £16,000 in savings capital or property he/she will be asked to pay a proportion of the weekly cost of the home. The Trust will subsidise the remaining cost. He/ she will continue to receive their state pension and attendance allowance (if applicable) towards the cost of their place in the home.

Where someone has less than £10,000 in savings, capital or property and the care managers' assessment shows that he/she requires residential or nursing home care then the Western Health and Social Care Trust will meet the cost of the home in conjunction with benefits (eg, state pension) available from the DHSS and Occupational pension.

When assessed as requiring Nursing or Residential Care then every effort will be made, by the Care Manager, to accommodate you in a home of your choice.

All residents in Nursing and Residential Homes will be entitled to a small weekly allowance, to cover personal items e.g. shampoo, sweets and clothing.

CARE PROVIDED

All homes will be responsible for providing personal care and supervision, as required. Nursing Homes will provide nursing care required. Foot care and Physiotherapy available on request through GP.

Laundry is provided by the home.

Besides your own clothes you are encouraged to bring some other personal effects, e.g. pictures and knickknacks that you value.

Visiting is not restricted and residents are encouraged to avail of any outings and visits to relatives and friends. Each home has a range of social activities available. Also hairdressing services are available at a low cost.

Chaplains of all denominations visit the homes. Services are provided by request.

MEDICAL REHABILITATION UNIT - Erne Hospital

The Medical Rehabilitation Unit for the Elderly in the Erne Hospital was opened on 1-2-89.

AIM

The aim of the Medical Rehabilitation Team within the Unit is to assist all patients to regain, as far as possible, their health and abilities, and to ensure that adequate supports, aids and services are offered on each patients discharge home.

STROKE UNIT

This specialist unit provides those patients, who have suffered from a stroke, with intensive therapy to regain their former skills.

RESPITE ADMISSIONS

If you are caring for an elderly relative who requires full time care your GP or District Nurse may be able to arrange short term admissions to the Unit as a period of respite. These periods can be booked in advance to accommodate family needs.

DAY HOSPITAL

Attendance at the Day Hospital is arranged by the Consultant. Its aim is to offer Rehabilitation for those living at home or being discharged from Hospital. Physio, occupational and speech therapy and a medical check-up are provided. Transport is available and the day runs from 10.00 am - 4.00 pm including lunch.

CARE MANAGEMENT

If your needs, prior to your hospital discharge, are seen as being complex and/or you will require a high level of community support and/or you feel that your needs could only be met within a Nursing or Residential Home. CHANGES TO FINANCIAL BENEFITS FOLLOWING HOSPITAL ADMISSION

It is important to notify your local DHSS of the date of your admission to, and discharge from hospital.

RETIREMENT PENSION

Continues to be paid at full rate for 6 weeks. Allowance book must then be returned to DHSS and will be reduced to the hospital rate.

INCOME SUPPORT

Continues to be paid for 6 weeks. The allowance book must then be returned to DHSS and will be reduced or ceased accordingly. However: a) Severe Disablement Premium is Ceased after 4 weeks. b) Carers premium is ceased after 4 weeks.

ATTENDANCE ALLOWANCE

Continues to be paid for 4 weeks. The allowance book must then be returned to Central Benefits Branch Belfast.

MOBILITY ALLOWANCE

Continues to be paid regardless of how long you remain in hospital.

INVALID CARE ALLOWANCE

Continues to be paid to the carer for 4 weeks. The allowance book must then be returned to DHSS.

HOUSING BENEFIT

Continues to be paid for up to 52 weeks providing that you intend to return home.

DISCHARGE ARRANGEMENTS

Following Your Discharge Home: Your GP will be notified by Dr J Kelly of your treatment and medications. You will be informed by letter if requiring an out patient appointment with Dr Kelly. Your District Nurse will be notified and will visit to provide any nursing care and discuss any nursing queries. Your Social Worker will be notified and will visit to ensure that necessary support services are in place and discuss any queries or problems. Your Care Manager, if appointed, will continue to visit to ensure that you receive the care in the community or in a nursing home to meet your needs. Physiotherapy, Occupational and Speech Therapy, Your Therapist will arrange an out patients appointment with you if required.

These are organised mainly by volunteers and are available in Enniskillen and in over 16 villages throughout Fermanagh. They offer social activities and outings. Lunch is available in some clubs.

SHELTERED HOUSING

In Fermanagh there are 9 units of sheltered dwellings which have been purpose-built by charity housing associations.

Each unit comprises of independent flats or bungalows for elderly and/ or the disabled, either for single people, couples or families. Each flat or bungalow is entirely self-contained, comprising of a bedroom, bathroom, living room and kitchen. All are centrally heated and, although unfurnished, each flat is carpeted and includes a fully fitted kitchen. Hairdressing is available on the premises and the Library provides a monthly service.

The great advantage of these flats is that, although they are self-contained, there is a resident warden, and, within each flat, bungalow there is a 24 hour call system in case of emergency. Tenants are responsible for their own household and personal duties but application for home supports may be made to the Social Services cases of need.

DAY CENTRES

The past few years have seen the development of a number of new Day Centres throughout Fermanagh.


These Centres are designed to help their members to develop, maintain or regain a fuller and more active life.

Application to attend these centres is made through the Community Services Elderly Care Team.

TRANSPORT

Each of the Centres offers transport to those unable to make their own way. This transport covers a radius of approximately 6- 10 miles.

MEALS

A Main Meal is offered, snacks are also provided mid-morning - early afternoon.

AIMS

To enable members to meet and have the opportunity to chat and exchange news. To provide personal care, such as washing and showering, if requested.

Hairdressing is also provided.
Foot care may be available.
Leisure and recreational activities, ie, craft work, indoor bowling, bingo, music and sing songs, are arranged and encouraged as regular features of the centres. Day trips are also organised.

CARE MANAGEMENT

WHAT DOES ASSESSMENT AND CARE MANAGEMENT MEAN FOR ME

Care management means that you should receive care suitable to your disabilities in the place you have chosen to live. It means that you can get a reasonable amount of help in your own home, taking your extra care benefits into account. If your needs are greater than this and you require nursing or social care 24 hours a day, then you can be offered residency in a nursing or residential home.

Your care manager will explain this to you and your family, and will help you plan your care.

WHAT YOUR CARE MANAGER WILL DO

With your agreement your care manager / social worker will assess your health and social care needs. She/he will do this by asking you to tell her/him your care needs, asking people who presently care for you to tell her /him your needs and their needs, and by asking your doctor, social worker and nurse to identify your needs. If you have been in contact with any other health care workers, eg, an occupational therapist or physiotherapist, then they too will be asked to assess your needs. Housing needs will also be looked at.

Your care manager then co-ordinates all these assessment reports so that she/he has a full picture of your health and social needs. Once this is complete and then depending on your care needs, you and your care manager will then plan the help and care you require.
This is called the care plan.

THE CARE PLAN

The care plan falls into 2 main categories:

(1) Community care provided to you in your home. OR
(2) Residential or Nursing Home Care.

In your own home there is a wide range of support available,

eg, Personal Care, Domestic Help, Day Care, Nursing Care, Sitting Service, Meals, Holiday breaks for you which also provides breaks for your carers, Convalescent Care, Rehabilitation, the supply of necessary equipment, assistance with adaptations.

Other ideas of help which you would give priority to will always be taken into consideration.

Once it is agreed that you need either residential or nursing home care you will always be given the opportunity to visit and decide which home you and your family prefer. All financial implications of this will be fully explained to you by your care manager.

CARE REVIEW

Your care manager / social worker then continues to keep in touch with you, visiting regularly so that you have the opportunity to discuss your care arrangements and ask for changes when necessary. You and your care manager will get to know each other well and your care will always be good even if not always what you desire due to, resource implications, the need for compromise, and the level of care you require.

Your care plan is monitored and reviewed on an ongoing basis and is then formally reviewed every 12 months. Your care manager is committed to responding to your needs and will always endeavour to meet them as far as possible.

RETIREMENT PENSION

Based on National Insurance Contributions.

ATTENDANCE ALLOWANCE

If requiring a high degree of personal care and are over 65 years. It is paid regardless of any other income, and is tax free. There is a high and low rate, depending on amount of care required. Application forms available from DHSS

DISABILITY LIVING ALLOWANCE

Claims must be made before you reach 66 years and it is paid regardless of other income or savings.
It is paid to those who need:
a) help with getting around
b) and/ or help with personal care

INVALID CARE ALLOWANCE

For those of working age who:
a) care for someone in receipt of attendance allowance
b) care for them for over 35 hours per week
c) do not themselves earn more than £50 per week.

INCOME SUPPORT

This is tested to supplement the basic pension and entitles you to apply for:
1 Severe Disablement Premium -all house hold members must be in receipt of attendance allowance with no one being paid invalid carers allowance for them.

2 Higher Pension Premium
a) If you or your partner or both of you are over 80 years

or

b) If you or your partner are over 60 years and are: Getting attendance or disability living allowance Getting severe Disablement allowance
Getting invalidity benefit
Registered blind
Provided with invalid care or receiving payments through invalid vehicle scheme.

3 Carer Premium -paid to carers who are entitled to invalid care allowance.

4 Housing Benefit -through the Housing Executive
Rent- up to 100% Rates -up to 80%

5 Social Fund Grant -to help;
a) towards travelling expenses to the hospital for visiting near relatives.
b) exceptional needs to enable you to remain living at home.
c) towards someone to care for you.
d) towards funeral expenses.

6 Travel Expenses for Out-patients Appointments - applications to be made through form available from medical records/reception, Erne Hospital. Please bring proof of income support i.e. Allowance Book claim made directly from Finance Department in Hospital.

CHANGES TO BENEFITS FOLLOWING HOSPITAL ADMISSION:

It is important to notify your local DHSS of the date of your admission to, and discharge from, hospital.

RETIREMENT PENSION

Continues to be paid at full rate for 6 weeks. Allowance book must then be returned to DHSS and will be reduced to the hospital rate.

INCOME SUPPORT

Continues to be paid for 6 weeks. The allowance book must then be returned to DHSS and will be reduced or ceased accordingly.

However:


a) Severe Disablement Premium is ceased after 4 weeks.
b) Carers premium is ceased after 4 weeks.
c) Higher Pension premium continues to be paid for 6 weeks if you are over 80 yrs. But is ceased after 4 weeks if you are under 80 yrs.

ATTENDANCE ALLOWANCE

Continues to be paid for 4 weeks. The allowance book must then be returned to Central Benefits Branch Belfast.

MOBILITY ALLOWANCE

Continues to be paid regardless of how long you remain in hospital.

INVALID CARE ALLOWANCE

Continues to be paid to the carer for 4 weeks. The allowance book must then be returned to DHSS.

HOUSING BENEFIT

Continues to be paid for up to 52 weeks providing that you intend to return home.

FOR THOSE CONSIDERING ADMISSION TO RESIDENTIAL OR NURSING HOMES:

If seeking financial support, application must be made to WHSSB. It will assess both the need and the ability to pay before placing and funding those people who need this type of care.

Drumhaw House

INTRODUCTION

Drumhaw House, a residential and day care centre is situated in a fairly elevated position approximately 1/3 mile from the main shopping area of Lisnaskea. The home has a pleasant rear garden and mature trees. The front of the building commands a magnificent view of the local mountains.

Drumhaw House comprises of a general residential unit, a specialist residential unit for dementia sufferers and a day care unit. Seven linked dwellings are located opposite the Home and are linked to it by a call bell system. The Health Centre and Library are also nearby.

OUR SERVICE

In Drumhaw House we provide a high standard of general residential care for 32 residents and a specialist residential service for 10 residents suffering from dementia. Our skilled staff provide a caring, responsive and helpful home life which promotes independence and affords all residents respect, dignity and privacy. We also provide day care, holiday breaks, convalescence care and meals to the local elderly population being cared for in their own homes.

We follow the six principles of good care, Which, are:

Privacy: The right of individuals to be left alone or undisturbed and free from intrusion or public attention into their affairs.

Dignity: Recognition of the intrinsic value of people regardless of circumstances

Their uniqueness and personal need to be treated with respect is of great importance.

Independence: Opportunities to act and think without reference to another person including willingness to incur a degree of calculated risk.

Choice: The opportunity to know and choose from a range of options.

Rights: The maintenance of all entitlements associated with citizenship.

Fulfilment: The realisation of personal aspirations and abilities in all aspects of daily life.

  1. We believe that residents require a home-like setting in which they can live with dignity, independence and in a manner respectful of their way of life.

GENERAL RESIDENTIAL UNIT

The general unit situated on the first floor accommodates 32 Residents. There are three lounges, a kitchenette, an ample bathroom and toilet facilities. A small lounge has been identified for residents who wish to smoke. All bedrooms are fitted with wash hand basin, hot and cold water, a call bell system and night-light. Central heating is installed throughout the building and a lift ensures easy access between floors. The main dining room is situated on the ground floor.

Residents are afforded the opportunity to enhance their quality of life in a safe, manageable and comfortable environment, with support and stimulation to help them maximise their potential physical, intellectual, emotional and social capabilities. They are involved in all discussions affecting their lives and they have opportunities to make choices about their own life-style, and
have their needs and wishes respected.

The objectives of this unit are in keeping with the philosophy and overall objectives already defined and will include the following:

  1. To provide a high level of care for each resident who lives in Drumhaw.
  2. To provide a range of long-term care services, short term and Day Care Services, Respite Care, and Meals-on- Wheels.
  3. To provide a caring environment, which promotes maximum independence and at the same time provides a safe place for eacl1 resident to
  4. live -thus affording the resident respect and privacy. To provide the essentials of life for the residents -ie. shelter, warmth, food security,
  5. recreation, social, emotional, physical and spiritual care.
  6. To promote a sense of worth and belonging for each resident.
  7. To assist each resident to achieve and maintain their aims or goals in life.
  8. To work co-operatively with other community agencies which provide services for the residents.

THE EMI HUMES SUITE

In 1995 we opened our 10-bedded unit for care of the confused but mobile resident. Nine beds are used for long term care and one for short-term care. Each admission will be for an assessment period in the first instance. All rooms are en suite and the decor throughout is soft and non-institutional. There is a separate open plan dining area and comfortable lounge.

We believe an essential part of the care programme is the correct assessment of the confused resident, provision of information
and on going support to carers and families.

Our aim is to provide a caring and calm environment suitable to the needs of elderly confused persons.

  • A good quality of personal care, flexible care and participative care.
  • A staff group who understand dementia care.
  • An assessment system to provide a base for planned intervention.
  • The enhancement of personal abilities, strengths and independence.
  • Care plans developed to suit individual needs.
  • A careful selection of resident with similar levels of disability to aid the design of programmes and the provision of consistent management.
  • The maintenance and development of family and significant relationships.
  • Support and Respite for, family carers.

DAY CENTRE

Our attractive and spacious day centre opened in 1993 and offers a full range of day care services to older people living in Lisnaskea and the surrounding area. This service offers support to people living alone, people being cared for by their families and people suffering from dementia or other illness.

DAY CENTRE OBJECTIVES

  • To respond to individual needs as identified by individual selves and carers.
  • To promote the full participation of users and carers in identifying means of meeting needs.
  • To involve users and carers in care planning and service planning.
  • To provide opportunities to use and share present skills and acquire new ones.
  • To provide appropriate therapeutic and recreational therapies
  • To provide opportunities to form new relationships.
  • To review care plans and services regularly.
  • To ensure that each service user's rights, dignity and privacy are respected at all times.
  • To ensure the involvement of family.
  • To respond to the needs of carers.
  • To liaise with and co-ordinate all other support services, when necessary.
  • To respond to changing needs offering different forms of care as appropriate.
